PLEASE READ - LIABILITY AND BE FIT POLICIES: 

I, the undersigned participant, understand that classes at Be Fit | Modern Pilates may be physically strenuous and are in a group setting.  I voluntarily participate in BE Fit activities with full knowledge that there is risk of personal injury, illness (COVID-19), property loss or death. I have consulted with my physician on my ability to participate in physically strenuous fitness and participate in a group setting. I hereby release Be Fit | Modern Pilates from liability and I agree that neither I, my heirs, assigns or legal representatives will sue or make any other claims of any kind whatsoever against Be Fit | Modern Pilates, its instructors or its members or heirs for any personal injury, illness, property damage/loss, or wrongful death, whether caused by negligence or otherwise.
I understand that the equipment can be dangerous if used improperly and I will follow the strict instructions given by my instructor with respect to use of the equipment.
I hereby acknowledge that I am participating in a group fitness class and therefore an an instructor may not be able to identify and make corrections to my form.

I understand that all personal property brought to Be Fit | Modern Pilates is brought at my own risk as to its theft, damage or loss.
I, the undersigned participant, expressly agree that the terms of release indemnity contained herein are intended to be as broad and inclusive as is permitted by the laws of California. Any provision of this Release found to be invalid by the courts having jurisdiction shall be invalid only with respect to such provision or portion.

 

Further, I acknowledge that I have read and agree to all BE Fit Policies as listed on the website at www.BeFitMp.com and understand that these policies may be updated from time-to-time, including:

-the class Cancellation Policy - Eight (8) Hours: Cancellations/reschedules of a reserved class(s) with less than 8-hours’ notice or not attending a scheduled class will result (per event) in either a $15 Fee or $25 No Show Fee (includes those reservations canceled within 90-min of the class start time), respectively, for clients attending via a 2-week Unlimited for NEW Clients or an Unlimited Membership.  For all other cancelations (class packages/membership) the scheduled class will be forfeited. NO EXCEPTIONS TO THE CANCELLATION POLICY

-Package/class series expiration: Classes/class packages and membership series have an expiration date.  No extensions are granted.
-Studio Schedule Changes: Classes and scheduled instructors are subject to change.
All sales are final: Sales of classes and merchandise are not refundable, exchangeable or transferable (see state specific information). Neither class packages nor memberships can be shared.
One New Client promotion per Client: A client is deemed a New Client if they have not scheduled and/or taken at a Be Fit | Modern Pilates location within the prior 18-months.
5-Minute Late to Class Rule: If you have made a reservation for class and are not present within five minutes of the class starting time, you will be marked as No Show and not permitted to attended the class; the Cancellation Policy will apply.
Children and Pets: Children and pets are not permitted in the Studio during class times.
